To receive treatment at the Center for Pain Management in Milwaukee, Wisconsin, you must be referred by your primary care physician (PCP) or a specialist. Some patients come to us with a prior diagnosis while others have not yet been diagnosed.

Before we can schedule your initial visit we need:

– A referral from your Primary Care Doctor
– A copy of your Wisconsin I.D and Insurance card
PRINTED imaging reports – this means x-rays, MRI’s, cat scans. These reports need to be pertinent to the condition you are being treated     for. We DO NOT take reports that are on CD discs.
– The last two (2) visit office notes from your doctor- INCLUDING your medication list
– If you have seen another Pain Clinic- their last two (2) visit office notes

This information may be faxed, mailed or brought into our office. We will not be able to schedule your appointment until we have ALL this information, and the information is verified by us.

Your Initial Visit

Dr. Nosheen Hasan will review your health history and conduct a thorough physical exam. She will want to hear all about your pain experience – where you hurt, when the pain began, what you think might have triggered it, and how it affects your life both physically and mentally.

Dr. Hasan will review any imaging studies other doctors might have ordered, such as X-rays, CT scans, MRIs, myelograms, etc. If necessary, she will recommend and schedule additional diagnostic procedures or tests to help determine the cause of your pain.

If you came to us with a diagnosis and Dr. Hasan concurs with it, she will develop a comprehensive treatment plan. Otherwise, this will occur at a subsequent visit.

Follow-up Visits

Once Dr. Hasan has made a diagnosis, you will begin the treatment(s) she recommends for you. Because pain affects you not only physically but also mentally and emotionally, we focus on treating you as a whole – both inside and out. Many people seek medication as a “quick fix” to manage their pain symptoms, but medication is only one aspect of our treatment process and is used primarily to improve function and your ability to participate in physical therapy – not cure your pain. That’s why follow-up visits may include interventional pain management treatments, a physical therapy evaluation and/or psychotherapy for behavior modification.

If you do happen to be prescribed narcotics for chronic opioid therapy, you will have monthly follow-up visits and will be subject to urine drug screens and serum screens as necessary. Refills for narcotics will NOT be authorized over the phone – you must come in to the office to be seen by a provider.

Narcotic pain medication can be an effective tool for managing and alleviating chronic pain. However, due to the nature of opioids, there is a strong risk for abuse and/or addiction if medications are not taken strictly according to your doctor’s instructions as well as frequent examinations and counseling. Federal law has strict requirements regarding the prescription of controlled substances for pain management: “All prescriptions for controlled substances shall be dated as of, and signed on, the day when issued and shall bear the full name and address of the patient, the drug name, strength, dosage form, quantity prescribed, directions for use and the name, address, and registration number of the practitioner” (21 CFR, Section 1306.05).
